• Hospital Annual Report (HAR): A 30-page formset containing over 500 hospital-specific data elements including revenue and expenses, staffing, utilization and services data. (See HAR Information and Education page.)
• Audited Financial Statements: Filed annually by Minnesota hospitals.
• Medicare Cost Report: Filed annually by Minnesota hospitals.
• Commentary: Comparative report of a hospital to its peer group showing raw figures and ratios.
• Hospital Profiles and Industry Profiles: Three-page hospital-specific and aggregate reports showing four-year trends of summary data by various hospital groupings.
• Ad hoc Reports: Custom reports from the HCCIS database.

Community Health Reports: These reports are based on UB-92 administrative data and are the result of a collaborative design effort between local public health, the Minnesota Department of Health, and MHA. There are three different reports, each prepared at the county level as well as the regional and state level:

• Number of Discharges and Utilization Rates for Selected Disease, Injury and Accident;
• Five Leading Causes of Hospitalization (Excluding Births) by Age; and
• Hospitalization Rates by CCHPR Chapter and Age Group.

Ratio Book: A report with over 25 industry financial ratios compiled using data abstracted directly from annual audited financial statements submitted to MHA.
